Jacqueline M. Slezak
Formerly Of Arlington Heights

Jacqueline M. Slezak, of Phoenix, Arizona for the last eight years, was born May 20, 1928 in Chicago to Michael and Anna (nee Rattay) Brunsfeld. She died Wednesday, November 5, 2008 at Gayles Manor in Phoenix.

Mrs. Slezak worked as a model in the fashion industry before becoming a homemaker. She later was a Certified Nurses Assistant at the Lutheran Home and Services for many years. She was an avid antiques hunter, a collector of recipes and loved horses. Jacqueline was a former member of the Arlington Heights Elks Club.

Her survivors include her children John (dear friend Lorraine Coyle) Slezak of Palatine, Judy (John) Johnson of Phoenix, Arizona and Jason (Lisa) Slezak of Barrington; her grandchildren Christian and Kayley; her sister Deloras (Jack) Watson and her brother Michael Brunsfeld.

Jacqueline is preceded in death by her parents.

Visitation from 4:00 until 8:00 PM, Friday, November 14, 2008 at the Glueckert Funeral Home, Ltd., 1520 N. Arlington Heights Rd., (4 blocks south of Palatine Rd.) Arlington Heights. Funeral Service 10:00 AM, Saturday, November 15, 2008 at the Glueckert Funeral Home, Ltd. Burial at St. Joseph Cemetery, River Grove.
