Jeffrey Dean Goldapske was born June 1, 1952 in Fond du Lac, Wisconsin, to Walter and Gloria nee Luby Goldapske, he passed away Wednesday, November 17, 2004 at his home in Buffalo Grove.

Jeffrey is survived by his beloved partner Diane Howland; brother Rick Goldapske and sister Peggy (Steve) Cohen.

A memorial gathering will be held Tuesday, November 23, 2004 at the Arboretum Club, 401 Half Day Rd. (at Buffalo Grove Rd.), Buffalo Grove from 5:00 PM until 9:00 PM. In lieu of flowers, memorial contributions may be given to:. Alzheimer's Assn. of Greater Chicago, 4709 Golf Rd., Suite #1015, Skokie IL 60076.
