Jerry N. Chapski was born on October 30, 1949 in Chicago to Joseph and Bernice (nee Hacker) Czapski. He passed away on September 11, 2008 in Arlington Heights.

He is survived by his wife of 39 years, Joanne (nee Damians); his children, Len (Deanna) Chapski and Stacey (Craig) Giacoma; his grandchildren, Joe, Ryan and Leah Chapski; and his brothers, Dennis (Barb) Czapski and Allan (Lynn) Czapski.

He was preceded in death by his parents and his brother, Robert Czapski.

Memorial visitation will be held Wednesday September 17, 2008 from 6 PM until 8 PM at Glueckert Funeral Home, Ltd., 1520 N. Arlington Heights Road, (4 blocks south of Palatine Road) Arlington Heights. Interment will be private
