Mark Chwaszczewski, age 38, was born July 4, 1968 in Chicago to Stanley J. and Helen (nee Mencel) Chwaszczewski. He died Wednesday at Northwest Community Healthcare Center, Arlington Heights.

Mark is survived by his father Stanley J. Chwaszczewski of Prospect Heights. Brothers Alan (Diane) of Boy River, Minnesota and Brian (Renee) of Wauconda. Niece Kelly and nephew Brian. He was loved by several aunts, uncles and cousins.

Mark is preceded in death by his mother Helen.

Visitation will be held Sunday, October 1, 2006 from 3:00 PM until time of Funeral Service at 7:00 PM at the Glueckert Funeral Home, Ltd., 1520 N. Arlington Heights Road, (4 blocks south of Palatine Road) Arlington Heights.
