Richard M. Slezak
Of Buffalo Grove

Richard “Rich” M. Slezak was born January 23, 1932 in Chicago to Albert and Anna Slezak. He passed away February 7, 2013 at the Brentwood in Riverwoods, IL.

Rich is survived by his wife Mary Lou (nee Clifford) Slezak; his children Richard M. Jr., Slezak, Jay (Teresa) Slezak; his grandchildren Bridget (Ernie Silva) Slezak, Samantha Slezak; his great granddaughter Sophia Silva.

Rich is preceded in death by his siblings Millie, Albert, John and Herbert and by his parents.

Visitation Saturday, February 9, 2013 from 12 PM at the Glueckert Funeral Home, Ltd., 1520 N. Arlington Heights Road, (4 blocks south of Palatine Road) Arlington Heights. Until time of funeral service 3 PM. Interment private.
